Different names on educational certificates and passport

Hi,

I got my ITA recently and I am quite confused if I should get a notary public to sign a one and the same person affidavit. My passport has "kumar" added to my father's name but my educational certificate from India does not have "Kumar". Should I get an affidavit signed or xxkumar and xx is assumed to be the same by CIC?

Please let me know.
