Hi,

I am applying for my wife's in-land sponsorship and was wondering if it is possible to have a different name on the forms than her marriage name. All the forms ask for her name on the travel document, and as they are pre-marriage, they have her older last name on them. For ex: In IMM0008, Is it possible to apply using that name, and only use her post-marriage name for question #2 (under Personal Details): "Have you used any other name?"

Your quick help would be gladly appreciated.

Yes, you need to use the passport name for the app and can list her married name as an additional name.

I used both last names, my maiden name and married name the exact way shown on our marriage certificate.
That was done on IMMO008 form under family name.
 
The first response is correct. You need to use your name as it appears in your passport.
