Declaration of No proof of employment/experience

Hi

I have over 8 years of experience in a MNC and the problem is my HR wont give me a experience letter for Immigration purpose. I found out that one can sign a "Declaration of No Proof of Employment" and get it notarized. So what all information should I provide in this declaration?
