Date on physical presence calculator

On the physical presence calculator form it says " Please make sure that he date of application used in this calculator and the date of signature on the application form and in this page are the same."

Does this mean the date when I printed this page(it shows at the bottom right) must be the same as the date on the application?

Not the date of printing. It is the date you put initially as to when it is signed. Based on this date, it automatically calculate your total presence days.
