Criminal Rehabilitation: which type of police clearance??

On the form for criminal rehab it asks for the police clearances of every country you've lived in since the age of 18, including Canada. My question: which type of Canadian clearance are they looking for? There are 3 types: Name based criminal record check, certified criminal record check, and police information check. Thank you!
