Could a Visa Office lose a package?

I sent a package to the Mexico City visa office containing my medicals, pcc, and payment....the package was signed by Daniel Reyes on July 24th, 2015. The visa office sent me an email saying they haven't received my package. What could be wrong?

If u have the proof that daniel has signed the courier, take a print out of that and send courier to VO or send him a scanned copy to his mail.
This is what we can do. or else u have to under go for tests again

Don't normally it takes 3 or 4 working to match documents with your file. Take a print of proof of delivery of courier and scan it. Send the scanned copy to your VO and they will find it.
