When you submit their PR application, use the 5476 representative form and fill out your information.

I'm using this form for my dad to get all his papers/correspondence sent to me in Canada.

wait, if you're in Canada with your sponsor where do you want the papers to be sent?

I think that you could just inform them of an address change and they will start mailing documents to your address in Canada.

Unless you want someone to mail the mail from your home in states to your home in Canada then I suggest you inform them of an address change.
