Checklist confusion

The checklist says "check for each applicable item"
So I have a confusion for the checklist as there are boxes after every number. And there are boxes of it applies to you.
So if I'm not appointing a representative, should I check the box beside number 11 as well as "I am not appointing a representative" box? Or should I just click the box that says I am not appointing a representative

You have to check the heading checkbox which means that you have attended this question.
Then, check the right answer.

Even if it doesn't apply to me, I still have to check the heading checkbox?
Read the checklist section heading properly. N/A should be asked to write in a case where the section doesn’t apply to you. For example, “Additional information about children” section. Tick the heading checkbox and tick or write N/A in its sub items.
