I used information posted by others on this forum to apply for CAIPS. I have posted the instructions i used so they are recent for those looking to apply as well.

Download IMM 5563B & IMM 5475E from CIC website. Links for forms are below:

Form IMM5563 : Access to Personal Information Form

Only these sections needs to be filled
1: Surname
2: First Name
3: Date of Birth
4: Access to Information ($5.00) - Only select Receive copies of the records.
5: Privacy Act(no Fees) Section - Only in English needs to selected
6: Request FOR- Demande Pour SECTION- Select only Caips
7: REQUIRED INFORMATION-RENSEIGNMENTS NECESSARIES SECTION - Only Embassy Name and Immigration file number to be mentioned
8: Request Detail by Canadian Citizen Section - Name, Phone Number, Address, Date & Signature of the person who is requesting CAIPS on behalf of you.


FORM IMM5475: Authority to release Personal Information to a Designated Individual

Only these sections needs to be filled out

1: Only Select " I Authorize Citizenship and Immigration Canada and Border......to the following individual"
2: Section (1-4) Give all your details: Full Name, Date of Birth, Embassy Details & Immigration file number
3: Section (5-6) All Details of your designated individual who is in Canada. Designated Individual can be your spouse or anyone who u know in Canada. Give all his details Name, phone number, mailing address, email address etc
4: Section 7 : Applicant Signature and Signature of your Spouse or Common-Law Partner


If you are in Pakistan or Afghnistan then the applicant should fill the form and send the scanned copies to the person in Canada so that after signing it he/she can mail it.


In the end the person in Canada should attach one draft of $5.0 with these document the cheque/draft is payable to the "Receiver General of Canada" . Also needs to be attached in the copy of his/her Canadian Citizen Ship Card or Canadian Passport Page of your spouse or Designated Individual with this application.

OR the sponsor can fillout form IMM5667 if he/she wishes to pay by a credit card

okay thanks alott ... is it okay tht my status is still APPLICATION RECEIVED or shud it b in process? my file was transferred to isb embassy on 12th of nov 2010 ... im reallly worried cux it already took 15 months n no reply 4rm dem ...

It's not abnormal, but i strongly suggest that you order GCMS notes and review them. I wouldn't worry as yet till I see the notes. Are you sponsoring your husband? Because husband cases usually take a bit longer to process.

Also, i would send an e-mail to CHC Islamabad and ask for an update. Pay close attention to their 'generic' response. If the response says that file is pending for initial review then it means that CHC hasn't even opened your file yet. If the response says that they are conducting background checks, then it means initial review was done on your file and now CHC is carrying out the necessary background checks on your spouse, i.e. some progress has been made on your file :)

how come everyone is gettin a reply back from CHC embassy
but its has been a two months now n they still havent replied to my email?

i send three emails all two-three weeks apart n yet still no reply?

WHAT THE HECK IS GOIN ON n also its has been a year since my file
transferred to CHC-ISL
im ording CAIPS INS'ALLAH this week n see

This response was sent back in December 2010 and surely they would not have reviewed your file so soon (as you stated earlier, the PR application was received by CHC in Nov 2010). I would suggest that you send them another e-mail and see what response you get from them now...

Based on few updates that we have received from the members, I know they are currently working on Nov 2010 files. So i think aap ka number, Insh'Allah, bohat jald anay walah hai :) Keep the faith...

The e-mail was dated December 2010 so it's not current :)
 
Dear Shamsia, Don't take those forum member so seriously as they just like to joke around with each other. The only reason behind their jokes is being lonely and partially being frustrated.

Mr. S.Jawad, Take a look at my timeline so you would know my application is for Oct see timeline under my name. I haven't received any letter yet. I am also waiting as some other forum member are ahead of you. It may take few days or couple of months depends on chc I.
