Hey guys, I hope yall can give me a hand with this, My wife recently got her new passport with my last name and we now have to change her PR card, here is where we are:

I want to make sure that I have the right documents on the envelope before I sent them out, but I cant open that link that gives me the document check list and also here is what we have as a way to double check:

  1. Photocopy of Current PR card
  2. US Marriage certificate ORIGINAL (we are both american)
  3. Photocopy of OLD US Passport
  4. Photocopy of NEW US Passport
  5. One photo that meets all the requirements (we took two but they only signed and stamped one)
  6. Form IM5444 signed and filled out
Questions:

  1. Is there a need for us to pay for this? if so I have no idea how to make the payment? (cheque?)
  2. Do we need an additional Photo?
  3. Do we need to send the ORIGINAL PASSPORT?
  4. On the link above i read the following: " Important information: If you have had a legal name change, you must include a copy of your Record of Landing (IMM 1000) or Confirmation of Permanent Residence (IMM 5292 or IMM 5688) with this application. " So I am missing this form as well right? this is what we had before we landed in Canada if im not mistaken, is that right?
  5. Anything else I am missing?
  6. Lastly, can she travel abroad with her new US passport and her PR card in the meantime?
