Hey guys, I hope yall can give me a hand with this, My wife recently got her new passport with my last name and we now have to change her PR card, here is where we are:

I want to make sure that I have the right documents on the envelope before I sent them out, but I cant open that link that gives me the document check list and also here is what we have as a way to double check:

I want to make sure that I have the right documents on the envelope before I sent them out, but I cant open that link that gives me the document check list and also here is what we have as a way to double check:
- Photocopy of Current PR card
- US Marriage certificate ORIGINAL (we are both american)
- Photocopy of OLD US Passport
- Photocopy of NEW US Passport
- One photo that meets all the requirements (we took two but they only signed and stamped one)
- Form IM5444 signed and filled out
- Is there a need for us to pay for this? if so I have no idea how to make the payment? (cheque?)
- Do we need an additional Photo?
- Do we need to send the ORIGINAL PASSPORT?
- On the link above i read the following: " Important information: If you have had a legal name change, you must include a copy of your Record of Landing (IMM 1000) or Confirmation of Permanent Residence (IMM 5292 or IMM 5688) with this application. " So I am missing this form as well right? this is what we had before we landed in Canada if im not mistaken, is that right?
- Anything else I am missing?
- Lastly, can she travel abroad with her new US passport and her PR card in the meantime?
