Canadian PR Visa to USA?

Hello,

I am a Permanent Resident in Canada (Edmonton, AB) and I have a Jordanian passport. I need to go to the US for a medical procedure. Do I need a Visa? Would I be traveling as Jordanian citizen or Canadian PR? I would really appreciate any help on where to start with this and how long does it take.. any details appreciated.
