Can I use a template for foreign work experience verification?

So, I checked out what is required to verify foreign work experience. I see that they're asking for an employment reference letter for every employer I worked for.

However, there is some stuff I was wondering about.

I created a template letter for my employers, I was going to tell each one of them to add their letterhead, their signature and some data that I need them to fill out. Is it okay to do this? Or does each letter need to be written by every employer?

I'm just wondering because I want to do this right. Also, does the date when these letters were issued matter? Or is everything fine as long as they present everything that's required?

Hi did you submit the letters in your formats? was it approved? and what about the letter dates?

I have referral letters but they are past dated as quite a few years back i was trying for Australian PR, would that be useful or I need to get fresh letters?
