In the case of medicals, I don't think anything is actually "sent" to the individual visa offices (i.e. snail mail). It is all done electronically. Basically the regional medical office actually analyzes and approves the request for extension, and then sends the VO an email saying the decision is made. The VO can then access the results electronically.
If you are seeing delays, it could also be due to the fact that the medical re-assessment is done but your VO has simply just not gotten around to re-opening your file again. Perhaps some VOs request the extension, and put the file on hold for a set x weeks while they go on to other applications.

Upon receipt of the request for reassessment, the MOF or the HB delegated staff will:
- Review the medical file to determine if there is sufficiently relevant and up to date medical information to provide a new opinion of admissibility on health grounds;
- Request additional medical information if a new opinion based upon the existing information in the medical file is not possible;
- Assess the medical file including any new additional medical information as needed;
- Provide a new opinion of admissibility or inadmissibility on health grounds;
- Enter the new ‘M' profile in the CIC electronic platform (such as GCMS or FOSS);
- Send an e-mail to the officer who requested the reassessment to confirm that the process has been completed
