can anyone say what does it mean

Police Clearance Reference form
- Filled and signed by reference Indian/foreign nationals
- Three copies from same individual reference with original signatures

does it means we need to fill Police Clearance Reference form
and then take 3 photo copies of it.

You can fill 3 copies of the form and sign them. Its a small form and should not take much time.
