Birth Certificate - Sample Letter

For birth certificate proof, can i apply an attested letter in the below format?

I, 'Name of FATHER', S/O,' NAME of GRANDFTAHER', R/o 'Address' do hereby solemnly affirm and state as under:-

That my wife namely 'Name of MOTHER', have delivered a male/female child on 'BIRTH DATE', at address.

My child name is 'MY NAME'

'The same information is also presented in tabular format'

This affidavit is being submitted for Date of Birth proof.

The above mentioned statement is true and correct to the best of my knowledge and belief:-
Sworn and signed before me
On this the 'date'
At 'Location'

Not sure if CIC accepts affidavits but I have seen people on this forum used to submit their secondary school certificate. It should have one of your parents name mentioned and your date of birth. Plus a brief letter explaining the situation.

Correct. Affidavit should be the last option. Secondary school passing certificate having father's name is accepted by CIC. I also had submitted the same and no issues.
