Birth Certificate - Change Of Name

Hey! I'm sure this is an issue with a simple solution but thought i'd ask.

I changed my surname when i was 16 so my birth certificate has a different surname to my passport. I have a document stating my name change from when I was 16, will a copy of this name change document be enough proof when submitting to CIC?

Yes. Make sure to also submit translations if the document is not in either English or French.
