Explanation

My wife’s two aunts are my mothers sister in law because they are married with my mother’s brother therefore my cousins from my two uncles are also my wife’s cousins from her aunts.

QUESTION, should i put the name of our cousins and our aunts and uncles?

My husband and i are not blood related

My wife (PA) mother and my father (sponsor) are cousins. My grandmother (my dad mom) is my wife mother's aunt.

So what to enter in "Relationship" column as this section has to be feel out by my wife (PA).

Should we write my wife (PA) mother name and Relations as Mother
Then write my father name and Relation as Uncle

You need to understand the difference between a cousin and an uncle. Your father is not your wife's uncle. She is his first cousin once removed and her mother is your first cousin once removed. You and your wife are second cousins.

She needs to write their relation to you, not to her. Include a letter of explanation if you think it is confusing.
