ATIP Number- ASAP PLSSSSS

Hi guys, this is an urgent questions to all that is online at this moment and knows about obtaining request to their files on process. I'm filling up the form for Access to information and personal information request. My question is where can I find and how would I know my the ATIP number? Pls ASAP response needed. Thank you a lot guys.... =)

1. Which do you prefer to recieve it in form? by hard copy or by e-mail
2. (This is silly but..) Since I am the requester of my own file GCMS, can I put my Driver's License number on the requester's reference number?
