Some Visa Offices have their own distinct procedure. Islamabad is one of them. You will get more informed advice by posting your query in the Islamabad thread. I am unsure what information you are asking for: in one place you refer simply to a medical and in another place to a re-med; and in any event I have no personal knowledge of the procedure at the Islamabad VO
