Applying for my father's visit visa electronically

Hi,

I want to submit my dad's visit visa application electronically. I am a PR in Canada while dad is in Pakistan. I have a few questions:

1. Do I need to submit the "use of a representative IMM5476E" form?
2. Should I give my contact number details in the form or dad's?

Any other helpful tips in my particular case will be highly appreciated.
