I got my application returned in the mail today and it looks as though these are the things noted to be missing.

- Submit recent photographs of the principal applicant and each family member. Please ensure that you provide the correct number of photographs (see Appendix A - The Document Checklist that accompanied the guidebook, "Immigrating as a spouse, common-law or conjugal partner, or dependent child") and ensure that the photographs conform to the specifications in Appendix C of that guidebook. In addition, on the back of one photograph (and only one) in each set, please write the name and date of birth of the person appearing in the photograph.

- Please review the guidebook accompanying the APplication for Permanent Residence and follow the instructions regarding a medical examination. Every principal applicant and their dependent(s) (whether accompanying or not) must undergo an Immigration medical examination by a Designated Medical Practitioner (DMP). The ORIGINAL Medical Report Form(s) (IMM1017 - Copy 2) signed by the DMP who completed the medical examination must then be resubmitted with this application package.

So, not going to lie, am a tad confused. Are they saying one of the visa passports my wife submitted is unsigned? As for the medical that cost almost $400 in Virginia, he sent everything in and didn't give her anything to take home at all.

Any suggestions on what to do next? Do they let you speak to the officer who looked at your package?
 
Hi Jeff,

The DMP should have given your wife copy 2 of IMM1017 on the day she had her medical, which should have been submitted with the original application. They are then able to match up the two forms to figure out which medical results belong to your wife (they have matching barcodes on them). I think you need to call the DMP your wife saw to try to track down copy 2 of that form unfortunately. Otherwise do you have any other proof that the medical has already taken place? Receipts etc?

Did your wife submit photographs of herself when you first submitted the application? I know I had to submit 8 photographs with my application, one of which had to have my name and date of birth on the back.

I am guessing your application has been returned by Mississauga before the sponsorship approval stage, is this right??

Ok to clarify, the second point has been solved. My wife has the IMM1017, the DMP told her to keep it for when she crosses the border and that the one needed was sent for her. So we messed up there I guess. So that part we have solved.

The first point we're assuming is the photos we need to send and one wasn't labeled with her name and date of birth (so that would explain that).

Has anyone that had to send their application back in have any suggestions on the best way to send it back? Looks like they gave us a label for it, can we not send it over night?

Yes, it was returned from Mississauga before the sponsorship approval stage.

She is sending the medical form by Fedex tomorrow.
 
Jeff,

I would send everything in the same envelope together (the application, photos, medical forms, etc). If you send things separately, it could take longer to match up your material together.

Yes, send it all together. Stick the label on the envelope and it will go on the top of the pile.

Sorry if I'm misunderstanding but I think you are misreading the passport photo info... ONLY ONE photo should be written on. The rest should ALL be blank. So maybe you wrote on the back of all of them instead of just one of them.

Hope you sort it out.

We didn't have information on any of them. My apologies, worded my reply wrong but many thanks for the clarification.

We're just relieved it isn't something huge. :D
