No no, just fix the error and re-send everything. You can either write in "to present" or the current date (nothing will be considered after that date anyway), or the date your work permit expires.

Test scores - dosen't matter that the envelope was opened, resend.

PCC - send the old one, with a note that you are applying for a new one.

Do I have to mention that note for PCC? Shouldn't they accept my old PCC?
 
Very sorry to hear about your rejection. I have told so many times to people, NEVER leave anything blank. Write N/A or Not applicable, where needed.

In your case the TO date would have been the expiry date of your work permit.

I know the case officer should have understood, he should have known by looking at your permit. But he did not. And now your case is returned :(
