Application forms for Open Work Permit, Tourist Visa Extension, Visitor Record

My Thai wife is going to apply for a permanent resident visa from within Canada, and we were told by an immigration consultant that in addition to the PRV / sponsorship application, to apply for a visa extension (she currently has a TRV), an open work permit, and a visitor record. The immigration consultant said that after getting a visitor record, my wife would be able to apply for Alberta Health Care.

So we're trying to confirm what application forms to use on the CIC website to apply for these:

PRV / Sponsorship: cic.gc.ca/english/information/applications/spouse.asp

Open Work Permit: cic.gc.ca/english/information/applications/extend-worker.asp

Visa Extension: cic.gc.ca/english/information/applications/visitor.asp

Visitor Record: ? (haven't been able to find anything about this)

Can you please let us know if these forms are the correct ones, and what form to use for the visitor record?

In addition to the above question, I was reading other threads here about not being able to send an open work permit application along with the PR visa application since they go to different places (PR to Mississauga, OWP to Vegreville). So should we just do the PR visa / sponsorship, visa extension, and visitor record first, and then do the OWP application after stage one approval? I still can't find anything on the CIC website about applying for a visitor record, so I'm not sure how it's done.
