You should send those file by Email to CPC-Ottawa.
MYCIC is only for Study Permit, Work Permit, Temporary Visa, and Express Entry applications.

The 4th line in ECAS system has been updated on May 20th, but I have not received my Passport Request yet. :( :( :(

Each line in ECAS usually updates after the CPC-Ottawa Email notification. In my case it is reverse!!!!!

What would be the reason of this delay?
Should I contact CPC-OTTAWA or wait?

Don't worry you will get your PPR soon. I have seen many people like you got 4th line updated before PPR. Don't bother yourself by calling CIC. It wont expedite your PPR. Just be patient.

I think background check is started after you pass the eligibility. No connection to RPRF n MR (unless you don't pay RPRF, report will expire)

Wondering..... does my wife need to do the medical examination again? - because she did it in September 2014 when she applied for work permit before coming to Canada to join me.

Minutes ago, received requests for CV and Supplementary Information Request for Students (SIRS)
It is weird that Iran is emphasized in the SIRS form...

May I ask couple of concerns about my case?
1. My wife took the medical exam in our home country when she applied for open work permit. The result was uploaded to CIC in 27 Aug, 2014. Now, does she need to retake the exam? Is what really matters whether the medical exam result will expire when landing?
2. The form of supplementary information request for students lists activities in Iran. But I have never been there. Leave these fields blank? Or state something related with corresponding activities in my home country?

I may answer your first question about the Medical test.

Medical tests are valid for one year. Therefore your wife's medical test is valid. BUT, CPC-Ottawa will issue your visa based on the medical test date. It means that your visa expiry date for landing would be 27 Aug, 2015.
If you do not receive your passport request by that time, your wife has to do the medical test again.

My suggestion: TO BE SAFE PLEASE DO THE MEDICAL TEST.
