That said, you may still include it. Are you able to get a combined one for each year you worked? I have never been in any other country than Hungary-Serbia and Canada in the last 10 years...list trips from:
Hungary to Serbia
Hungary to Canada
Canada to Hungary
Canada to Serbia, etc in the last 10 years or since your 18th birthday if this was less than 10 years ago.

I have a Philippine passport but I renew it here in Vancouver .What country should I use in the question country of issue?Philippines or Canada.

Country of issue = Philippine. Here's what the guide says:

Passport
Question 3 From the list, select the name of the country that issued your passport or travel document.

Many many thanks for this section. Exactly I am looking for this.
My question is on:
1. Additional Family Information (IMM 5406)
This form must be completed by:
You, as the principal applicant,
Your spouse or common-law partner (whether accompanying you to Canada or not), and
Your dependent children aged 18 or over (whether accompanying you to Canada or not).

I have filled this form as Applicant now for my Spouse, I get confused on starting row as if I put my spouse name first then next column says Applicant(which is fixed).The next row is for Spouse where I put my name so no confusion there.
Please tell me what to do.

However, as I mentioned before I've travelled 2 times per weeek from Serbia to Hungary and Hungary to Serbia. Which means 8 trips per month, 96 times per year....Nobody can track this...I can't put 800 times for 8 years...this is ridicoulus...I don't know how to resolve this issue....:-(

family information form should be fill up by you as principal applicant, 1 form for your spouse and another form for your child who is 18 yrs old or over ..you can fill up the form but they should affix their signature also..
 
You may group the trips by month. e.g. if you took 8 trips in April,
"from" = date of first visit in April
"to" = date of late visit in April, ... etc for the other months. Include a comment to say you took 8 trips per month and grouped them by month as they are numerous.

This is a requirement and you just have to do your best. People may not remember every trip they've taken but every effort should be made to provide the information truthfully. Hope this helps!

I am little confused about Statutory Declaration and Common-Law Union (IMM 5409)

Who must complete this form?

This form must be completed and signed by the sponsor and common-law partner, only if the latter is a co-signer on the application.

For my case, I am the principle applicant. Me & my wife living together for 6 years. No one is sponsoring me.
Do I need to fill this form (IMM-5409)?

Please advice.

Only if you are not legally married to your wife. If you are living together for at least 1 year but not married, you care considered "common-law" partners/spouses. If you ARE legally married, you do not need to fill out this form, but you do need to provide a copy of your marriage certificate.
 
on Schedule8 form Q.7 Education in Canada. Do I have to put only Diploma courses or include EAP course too?
