Will Marriage certificate work as a proof of name change? Though it does not have my name before marriage but its does link me to my father?

Can you please suggest a format of the name change affidavit due to marriage. Do I need to get it from India?
By chance do you know the process to get Affidavit in US as I live in US.

There will be 2 separate Placeholders one for marriage and one for legal name change document.
I am afraid i won't be able help you on how to get it from US, I got it done from India.
Following is one of the sample template

I, [post marriage name], D/o [father’s name], wife of [husband’s name], current resident of [address], do hereby solemnly affirm and declare as under:-
1. That the deponent is the current resident of the above said address.
2. That previously the name of the deponent was [maiden name] and after the marriage of the deponent with [husband’s name] the name of the deponent was changed as [post marriage name].
3. That the new name of the deponent i.e. [post marriage name] will be used by deponent for all purposes in future in the Government, Semi-government, Non-Government any other sectors.

Thanks Vishal this helps, I will try to get it from India. But now another question what should be my address as I don't live in India any more. Not sure whether lawyer will be able to verify that.

Vishal my wife's name is also changed after marriage to update my name in place of her father's. If this really required to have name change document? Well I never thought we may need such a document..

In the document checklist they asked for "Legal Name change document". I checked around, people suggested to get this affidavit done.
If you are in India, getting this affidavit is very easy. Not sure about other countries and the legalities involved.

Can anyone please advise what should be the "Field of study" equivalent for Indian B.Tech (engineering) degree as I don't see engineering as an option in drop down? Should it be Applied Science?

You can also show smt like an old passsport with her maiden name on it.... its a government document and it is acceptable to my knowledge...
 
Hi guys,

I am bit confused, can someone please help me on the below two queries?

1) The "Personal history" and "Work history" section should be sorted in the order from Newest to Oldest? I read somewhere that it is a must, but double checking as it would need quite an effort again.

2) My wife is the main applicant and my work experience is outside Canada. Two of my initial employers had my name in their records different than my Passport name and name as per my latest two employers [x x Goyal instead of my passport name as x x]. Should I remove these experiences from work history and keep in personal history to avoid any issues around the name? Or should I get an affidavit to declare both names for me? Really confused.

field of study is related to your education and it's correlation with your work experience isn't of importance (at least with Canadian PR process)

I am sorry, I am not aware of the options that can be used for the spouse. May be someone else can help you out here.

Employer B - 1 year with NOC Y

I am currently working for Employer B and I do not wish to claim points for that. so do I have to state that in the EE profile? can I just ignore Employer B? will this be a problem as I would need to explain the reason for unemployment if I do not disclose employer B???

you can ignore employer B. You will be making NOC X your primary NOC.
Once you get an ITA, you can put experience at organization B in personal history section
