Your medicals should not be older than 6 months when submitting your PR application. This is what is mentioned on the IRCC website -

Medical examination confirmation
Purpose

To assess the health of the applicant and their family members
Document requirements

The medical examination confirmation is required upfront at the time of application and must be valid for a minimum of six months.
The applicant must include the immigration medical examination (IME) number in their e-APR.
The applicant must provide a copy of either the information printout sheet or the Upfront Medical Report form [IMM 1017B], whichever the panel physician provides upon completion of the medical examination.
Exceptions are made only in rare circumstances that are outside of the client’s control (e.g., cancellation of appointment by panel physician at last minute without time to reschedule).

If your medicals is not more than 6 months old, then yes, you can use it. But I don't recommend it because it will give you no practical landing time after you get your visa. You will have to land in Canada before your medical expires.

Does the job have to be in one occupation only ?

When they mean one occupation (or NOC), it is with regard to the basic requirement that you have worked in one NOC for not less than 1 year (in the last 5 years). Otherwise it does not matter how many different NOCs you have worked. However for the purpose of point calculation, all your foreign work experience will be counted. I myself have 3 NOCs, I don't think my score was affected.

My medical is passed and they asked me schedule 4
Is this means my eligibility review is over ??
And all criminal record check too??

No you do not need to use a new email address to create a new profile. Withdraw old one by logging into your current MyCIC account and then create a new one.

I have to prove my relationship with my uncle. So i have his and my mother's birth certificates as well as mine.
Problem is that in my birth certificate my mother is listed with the family name of my father (which is the same as mine). However, she now has a different name because she re married.

What other documents do I need to attach to explain this?

Along with the above mentioned documents, you can produce the marriage certificate of your mother to prove the name change was because of marriage. You might also consider getting an affidavit made from a competent authority that says the name change was due to marriage.
 
Marriage certificates in my country include the statement "after marriage ..someone ... will be known by the name .....someone.." so affidavit is not a necessity I think.
As for the marriage certificate of my mom - do I need to include both to explain her name in my birth certificate and also the name in my current application where I list my relatives?

Yes, makes sense to submit both. In a LOE, explain your situation.
 
Can anyone please help me answer this question..?
So my partner just got his work permit renewed the problem is Cic made a mistake in his job occupation he has a positive lmia for assistant manager (noc 0621) when his work permit came the job occupation said "head cashier" (noc 6211). In the conditions at the very bottom is the lmia number used to process that work permit which is the lmia number from the assistant manager one, so cic did make a mistake on that.
So we had that sent out to Ottawa for the amendment.. Its 2 weeks already...
My question is how do i got about answering the express entry profile question for "was the job offer stated on the work permit?" do i answer no?
Anyone suggest please what i could do :)
