Any December 2014 TRV applicants from within Canada and still waiting

We applied on December 15th 2014 for TRV out application was opened on 7th of January after that there is no status update .our travel date is 16th of Feb 2015 so submitted the withdrawal of application today as the withdrawal time line is 10 business days.
We would be applying the TRV from Delhi. It takes less than a week from there. Applied for my mother in law on 26th of December in Delhi and her TRv got approved on 31st of December.

When I'm filling the application I just confused by some questions such as :

Do I need proof that by submit a copy of my IELTS repot ??

I don't have any person in canada can I write N/A ??

Please check this link because i checked the application kit for visa office at Cairo and i found that visa office require this form but there is no option for print the form
