Today I got Medical & Additional Info. can anybody suggest me where I will send all additional info.??
They asked for IMM 5669. Do I send my 8 months old child's IMM 5669??
"Personal History" of IMM 5669 there are only 5 Row but I need more space to do all. how can I extend row or column

nobody can help me ??!! I have only 40 days...Plz give some advice

No, they don't send any blood sample. They'll test blood, X-Ray, Urine, Blood Pressure, Eye sight, Hight, Weight and a bunch of question about any past medical history.

can anybody explain me the word "travel documents" ?i mean what docs i have to carry at airport?once again about cell phone.if i take any GSM set with me will it work in canada?

Smiling_face has already given you some guidelines about it in earlier page. Pls check and if there are still ambiguities dont hesitate to post inquiry !

1) Have you already added your 8 months old kid in your application before getting MR or yet to add ? I am not clear !

2) Download Recent Application Guideline from cic.gc.ca in order to fill up those forms. Go through those specific pages!

3) if space is not sufficient in any row & column, use additional page with that particular form.

4) These dox to be sent in the same address where you sent your full dox to SGVO earlier (See address given by smiling face)

Already I have added my child info on May'12 before getting MR.
can you paste "smiling face" 's advice here ?? Do I need my child's police clearance ??
Do I sent all supporting docs. with IMM schedule ??
If I update my passport to machine readable from manual then what to do or is it risky ??

GSM shouldn't be any problem, you might need a converter for the charger (easily available in canada, you can use universal charger as well) and some say cdma operators has cheaper call rate in canada. so, you can keep cdma in mind.

CONGRATS!!!

The updated documents/forms need to be sent to:

IMMIGRATION SECTION
HIGH COMMISSION OF CANADA, SINGAPORE
P O BOX 845
ROBINSON ROAD
SINGAPORE 901645
FAX: (65) 6854-5874

You need not to bother with the medical documents. DMP will send the documents directly to the right destination (Makati City, Manila, Philippine).

Please read carefully the letter and instruction in the form IMM 5669. It is clearly written that this form need to be filled up for the children aged 18 years or more. So, you DO NOT need to submit it for an infant.

If the 'personal history' exceeds 5 rows, you can use blank page (as attachment) at the end to accommodate the continuing/remaining information. Please make sure that you are writing the 'name of the applicant/spouse' on the top of the attachment and also mention the 'question number' you are answering.


N.B. When you will send the RPRF, please write Principal Applicant's name and File Number on the back of the Bank Draft. Many people forget doing so.
