Today I got Medical & Additional Info. can anybody suggest me where I will send all additional info.??
They asked for IMM 5669. Do I send my 8 months old child's IMM 5669??
"Personal History" of IMM 5669 there are only 5 Row but I need more space to do all. how can I extend row or column ???

I already did for my spouse, and send a CSI with a scan copy of her MRP last week. I have good experience in using MRP abroad as it gives less hassles in immigration counter. In anyway today or tommorow we have to do MRP then why not today. Even if you go to canada you will not be able to do MRP as this facilities are not available yet in Bangladesh missions abroad, so better to do it now before we receive PPR.

As medical request are mostly sent through regular mail, but due to poor service in Bangladesh postal offices, sometimes the mails are not delivered on time and missed also. For this I want to know from your office if medical request letter was sent or not and if not can I have any approximation when it will be send so that I can be assured that I didnt missed the mail from the post offices.

The updated documents/forms need to be sent to:

IMMIGRATION SECTION
HIGH COMMISSION OF CANADA, SINGAPORE
P O BOX 845
ROBINSON ROAD
SINGAPORE 901645
FAX: (65) 6854-5874

You need not to bother with the medical documents. DMP will send the documents directly to the right destination (Makati City, Manila, Philippine).

Please read carefully the letter and instruction in the form IMM 5669. It is clearly written that this form need to be filled up for the children aged 18 years or more. So, you DO NOT need to submit it for an infant.

If the 'personal history' exceeds 5 rows, you can use blank page (as attachment) at the end to accommodate the continuing/remaining information. Please make sure that you are writing the 'name of the applicant/spouse' on the top of the attachment and also mention the 'question number' you are answering.


N.B. When you will send the RPRF, please write Principal Applicant's name and File Number on the back of the Bank Draft. Many people forget doing so.

nice csi, it is important to emphasize on 'letters get lost in post offices' issue while making query about MR (as suggested by my consultant)
