If you have anyone in Canada the you have to give this consent form to him. Before that you have to provide his e-mail address to the consent form. After filling up the form take a print of it then sign it. Your sign should be as per your PP. Then scan it again and send to your friend. Your friend/relative will then apply online(the link that i have provided). He has follow it step by step. When he does make sure that you are also in communication with him as he has provide info regarding you. What you can do now is, you can click the apply online and fill it up step by step. I mean like a mock test. So you'll know what is in there. I hope rest all you will understand and can do it by yourself. And your friend/relative will have to use his credit card.

can I use my credit card and my email address for this?

yes u can. :) i did it brother. i applied in the name of one of my senior who is living there. and he didn't know anything about it. I just applied using his name, address and cell phone number. just after 30 days i got my GCMS note in another e-mail address of me (should not give e-mail address which u used in Application)

That's good. But i wonder what documents you provided to prove that you(Your senior) are residing in Canada. Specially when you said that your senior didn't know anything about it.

Hmmm.....But when i applied for GCMS through my brother he submitted his study permit(scan copy) as without it the system wasn't accepting. And in the system a format was there to upload all documents including his to prove that he is residing in Canada. And i myself did entered all his details and in one point i got stuck cozz i have to submit documents to prove residential status. And it is still there. That's why i asked. Next time i'll do the same as you did. I hope it works. :)

WOW....that's a new information. So lets check what CIC has told us to do...........

http://www.cic.gc.ca/english/pdf/kits/forms/IMM5563E.pdf

See point number two. And pls can you explain how you filled up the second form section B point 1? And as you said you did it i would like to request to help mr Afaruq and others so that they wont have go for a hassle. Without providing any documents regarding PROOF OF RESIDENCY you did it. If you can do it, others can do it.

I would like to request other applicants to do the same thing and provide us your feedback. Cozz it seems that only in my case i have to provide PROOF OF RESIDENCY even though my uncle and aunt are the citizens. Brother is in study permit. Cant get it done only by providing their names, addresses etc.

I used this form (below link) for consent.

Below link is the sample review page of the GCMS request

Below link is the proof that I easily go through to payment form without giving any sort of proof of residency in Canada

And i used below link for GCMS note request directly from CIC

Hi seniors,

I need an advice ... couple of months back , my passport fell in to the bucket of water ... I recovered it after one day ... The condition got bad , even after i left it to dry ... however, the passport is readable ... and I have traveled to couple of other countries as well , using the same passport ... The immigration officers asked me if it was being washed or something ... I cleared their doubts and I was good to go ... Now advice me , if I apply for a new passport or the previous one is fine ? Since, At the end CIC may ask for the passport for issuing the visa ... so I was bit worried regarding this thing that what should be done ? However, I am at the stage when CC gets charged ... so I am waiting for PER/NER ...

[size=10pt][size=10pt]Please Advice !!!!!

I heard today like my case where medical request has
Send but no received at ecas or delay of passport
Request they do not provide feedback
Is it right info?

You have to go a long way.......PER, AOR, MR, PPR.....
After your passport request you may change your passport and send the new one.
