I sponsored my spouse for permanent residency from outside. She is currently in Alberta on a TWP that expires end of the month. Her employer is applying for a new LMO but we do not expect it to be approved until sometime next month.

She has her Alberta health card that expires with her TWP. If I apply for a card as the primary, can they extend her card?

There is no primary or secondary on a health card. Everyone applies for and qualifies for their own.

Have a look here http://www.health.alberta.ca/AHCIP/registration-requirements.html, where it specifies:

All eligible residents of Alberta must register themselves and their dependents with the Alberta Health Care Insurance Plan (AHCIP)

Married spouses – must register together


Your wife is legally your dependent and by AHCIP's own rules, you must register with her. However, her imminent status expiry may cause some issues.
