I am sponsoring my wife from Vietnam. Her english is limited and also cannot write english that well. I am helping her with her application. I am doing most if not all the work. In my application where it asks on a separate sheet of paper provide any additional info. to prove relationship is genuine....I have already wrote a detailed letter of our relationship explaining every aspect. My question is on my wife's application where it asks for the same thing on a separate sheet of paper, does she really have to write anything? Will it look bad if we leave it blank? Like i said, ive already explained and provided everything in my application.

You are going to have to provide proof such as pictures, phone/text records, gift receipts, money transfer records, proof of visits (boarding passes, exit/entry stamps), etc. Basically anything that can be used to prove geninuity of your relationship.

Yes. I have already provided everything that you have mentioned and many more evidence. Should my wife also write a letter explaining our relationship? i have already explained everything in my letter. She is basically just repeating what i have written in mine. Or does VO wants to hear the story told from her point of view? She cannot write english well, so will prolly need someone to help her write it.

Why don't you help her write it? It should be clear she is receiving assistance from you, so make sure you state it somewhere. It doesn't have to be lengthy, just make sure it's her words and her thoughts and feelings, and all you do is help her put it in English.

I would think it's a good idea to have a letter from both of you, as mentioned above it would be good to show both points of view. We did this as well. My wife's letter was 15 pages long, but mine was only 4.5 because I felt she covered the bulk of it in her's but I still wanted to tell my side of the story.
