Additional doc request

i got additional docs email, my file reached July 7......the email says that they need following for my wife but i am confused.... my wife is in Canada and she had medical for work permit in Feb 2014, so can you please tell what they want


it states

UCI-NAME

Medical: Please provide an updatesd residential history for the last two years.




So what i need to submit?

i understand that they want her residential history and can you please tell which form i need to submit and apart from that why it says medical there and then residential history...do i need to submit anything regarding her medical that she had in feb 2014....
