Quick questions about this last statement on IMM5490:

Does this page has to be attached as the last separate sheet of the said form or, can this be put as sort of a Cover Letter? If to be attached, I should then just follow the instruction of putting my name, form title and the boxed statement?

Sorry if you find this post weird but I am just confused Hoping to get feedback from anyone. Thanks!

Hi, Survivor. Yes, it has to be attached as the last separate sheet for the Sponsored Spouse Questionnaire Form. And yes, you should put your name, form title, and the boxed statement. But as for me, I used the title "Additional details that would prove our relationship is genuine and continuing." I'm pretty sure you can use the boxed statement..
