"academic records request form" in the sealed envelope for WES

I have a ridiculous problem. I recieved my academic transcript without"academic records request form" and since it was in Persian so a certified translator translated it to English
I asked the university to sealed the envelop but they reject my request and told me we are not allowed to sign any english letter such as"academic records request form".
So they just can seal the academic transcript
is it necessary to put "academic records request form" in the sealed envelope for WES?

As long as you have got a sealed envelope with the transcript inside, from the university folks, this should be fine.
However, i would urge you to add a letter stating the same in your envelope and it should go well.

Unfortunately my academic request form )signed and sealed by univ exam controller) is not put inside the sealed envelope? What will I do now? Can I send the academic form separately in a different envelope?

You should be fine. Happened with me too. I called them up and they said just put your name and reference number on the envelope. Put the academic request form in the post along with sealed envelope and you're good to go

Request form is not required if your university agree to send the transcripts only. I called the WES contact number and they assured me that request form is not mandatory.

I did the same and recieve the WES report without any issue.

I did not use the academic records request form for my Indian education and my for my wife's Indian and UK education. My UK university requested me to send it but I filled it only because the university and not WES asked. Our WES status right now is evaluation in progress. This means WES have accepted our documents without the academic records request form.
