A few questions on extension application?

1. Will my application be returned if I checked both Never Married and Common Law?
I checked both because I haven't ever been married and I am in a common law relationship. Maybe I am mistaken.

2. Do I list my common law partner in the family member list?

3. If I do put him down, where it says given name(s) should I put his real first name or would it be alright to put down the name he goes by? Or should I just put both his first and middle name?

ALWAYS USE NAME ON PASSPORT.
NO THEY WANT TO KNOW IF YOU HAVE BEEN MARRIED BEFORE THIS RELATIONSHIP
AND HE IS NOT FAMILY MEMBER LIST HE IS PERSON BEING SPONSORED
THE FAMILY MEMBER LIST IS FOR OTHERS

He is the sponsor not the one being sponsored and on the application guide for the extension it says family members are your spouse, common law partner, and dependent children.
