Apologies for the vague title but I have a couple of small questions I'd like to be sure of before mailing my application...

As I am applying for renewal, do I put my previous UCI number?

Question 19/20/21: For all dates up to the present day, do I just leave the final "To" date blank? (e.g the "To" date in my current job or address)

Question 21. in the "period to be assessed" the "To" date....do I put the date I am mailing the application?

Do I need to fill out the Supplementary identification form? Why or why not do I need to do this?

Question 19/20/21: That would likely be acceptable. If you want to make it obvious (and have less risk of problems), put the day you are applying.

Question 21: Yes.

I do not see the supplementary identification form you are referring to.
