IMM 0008 Application: A completed Generic Application Form for Canada (IMM 0008). This must be received at this office by: 2019/06/14
Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature. This must be received at this office by: 2019/06/14
Additional Family Info IMM5406: A separate and newly completed Additional Family Information form (IMM5406) bearing original signatures. This must be received at this office by: 2019/06/14
Police Certificate: From each country/state where the residence period has been six months or longer since the age of 18, provide an original police clearance certificate. This must be received at this office by: 2019/06/14

They probably lost it they dont even know wats happening cas they dont touch the file till they pass one year lol they only look at it when jast 4-6 month left lol
I am applying Paper based application and on the very first IE 0008 Generic information form, It ask for a first an last name that is Family name and Given name. Now, I don't have a Last Name on passport and my whole name is written in Given name but the form does not let you let it empty or put N/A. Can somebody help?
That is the interesting part, My passwport says Family name : Blank and Given name : XY
and when you try to leave Given name blank, it wont allow you to validate the form
Xy is your first name so leave the last name blank if it doesnt validate then put y as last name wats your name ill explain it much better
I assume that First name is Given name and last name is family name, so i dont carry my last name (Family name) onn passport but in generic information form, if you leave Family name blank, the form will not be validated
