For the first one, sorry I don't have idea which insurance is good for SUPERVISA , but check this thread

For the second one, I just told the clinic that we are doing upfront medical for visit visa extension. Just tell them exactly which one you are doing whether it's for SUPERVISA or just regular visa. I believe they are the same since they accept ours and also the other forum member who had done SUPERVISA medical for the PR medical. Just keep the e medical receipt they'll give you and inform CIC when they request for medical, although in my experience, they didn't even asked about the emedical receipt after I informed them. They just said disregard the request.
