Best is to be honest with all answers. I don't think any condition which is stable for 4 years will cause any problem. Your father could also go and see his own kidney specialist and get an update of his condition which you could provide the panel doctor when he has the medical examination for PR. This will show that his condition is stable and no cause for concern.
Are you doing upfront medicals or are you waiting for the email.

eMedical-enabled Panel Clinics

Panel physicians must first search eMedical for the client by their name and identity document number (e.g., passport number) to confirm that the client does not already exist in the system.
If the client does not exist in eMedical, click “create case.”
Enter the Client personal details (mandatory fields include Title, Family name, Gender, Date of birth and Country of birth).
Enter the Identity Document Details. Clinic staff will check identity with the approved identification document. (Note: a live photo of the client must be taken and uploaded to eMedical at a later step during the IME.)
Enter the Client's Preferred Language.
In the “Proposed Visa Application Details” section, select the appropriate upfront category from the drop down list (student, worker, visitor or family EDE). Note: For refugee claimants in Canada, select the category family EDE, and scan and attach the document provided by the client.
Click “Create.”
Proceed to upload and attach client photograph.
Complete Identity Confirmation.
An “Information Sheet” letter will be system generated and must be printed and given to the client for submission to CIC along with the visa application.

I was asked to do upfront medical. My parents will do that and hopefully the process should ease. however I have been waiting to hear from London for the last month or so. I suggest your parents do the upfront now, as they are asking for this in the approval letter.

I know our relative who immigrate to Canada was taking insulin, heart disease even then he got PR. When my father was referred for additional test for chest last year at Mohali then the penal Doctor informed my father that CIC does not allow person who has communicable diseases like Ebola, TB. HIV/AIDS Rabies etc, (You can browse list of communicable diseases on Goggles)in order to ensure that canadian citizen should not be affected from these communicable diseases. Even when my father chest xray was good as per Indian Doctors but due to some stains in the chest my father undergone for 3 month test and at last he was cleared by Canadian Doctors. Rest it is upto you to decide. I had a point that too in my fathers case I have narrated you. I wish for clearance of medical of your applicant. Rgds

I remember when we were also processing my son's spousal sponsorship of his wife, they also received a confusing email from CIC Manila and the email says something like this "at the bottom of this letter, you will find additional documents required in order to continue processing your application." But we cant find anything listed at the bottom, so I told my son to email CIC manila and ask if there are additional docs needed or nothing at all. In fairness, after two days they replied that there is nothing they require

I have filtered out only those that have not been active for April/May. It is impossible to guess what stage of processing those members are at. However for active members we can see that more than half the files at Ottawa have received medical requests.
Unfortunately the spreadsheet is only accurate if everyone is active and updating it otherwise it begins outdated in 2-3 months. I found 7 members who had not been active in just the above list which I have edited today.

1. Fill the form to incluede you as a representative, so all the communication comes to you directly.'

2. I have created a new email address solely for this purpose and provdied to CIC. This ensure it does not get lost / mixed up with the other email that come our way.. Below is the link I have a, in case someone is looking for it.

Yes we are in same situation along with Kaitmhia (Jan 9) nitinbali(Jan 28)parentsimm(Feb 26)and you Chotis(Mar 19).
We seem to be the only ones waiting for medicals from Ottawa. Maybe we will all get emails together around the same date.

Just one Quick Question. I am June 2011 Applicant. I know they are going to request option-c printout soon. Last year i got for 2011 and 2010 in advance. should I get new fresh when they ask or it is okay to send them 1 year old. Please reply me anyone got in advance or has idea on this topic. I am trying to collect all papers in advance before thy ask.

While it won't make any difference the the printouts are 1 year old as long as they are original. however if you want then at beginning of July ask for new option c printout for those years and also 2013, 2014 in case this years they ask for different years.

last year we were contacted at end of July to submit documents. Also consider police certificates which your parents can get around end of July and you can send these too.
