    Application is still in process and yes, it has a stamp at the back from the photographer mentioning the address and date the photo was taken including my name and DOB.

    The photographer provided me with both digital and printed photos and instructed me to upload the digital photo for the front and a scanned copy of the back of the printed photo for the back.
