There's no certain way to know. Maybe it will trigger someone to look at your file, maybe it won't. Doing upfront meds has made this happen for a few people, for others - not. It's all a matter of luck really, no hard and fast rule about it.
No, don't do re-medical if you are not requested to do so. I read info from an immigration lawyer about this. When IRCC tells you to do a re-medical, they create a special IME number that will trigger their system immediately the moment the results are submitted. If you do upfront re-med, they won't see it for months.

Here is the thing. Ottawa in theory is processing March-may/20 aors and then some random late 2020/ early 2021. Today in my WhatsApp group was a good exemple. We had 2, one March/20 who was requested remed while one nov/20 who did upfront. So yes, doing upfront will take longer for an officer to look at it, but as Ottawa is concerned, after they look at it, they tend to work on it. Whereas if you wait, it might take longer for them to request you a remed, then it would take for them to look at you file after doing upfront.
