Hi everyone. Is it ok if I write "see attached sheet" on every single section of my travels on the form and then create an additional sheet where I write all my travels OR I should only write "see attached sheet" on the last section, fill first ones up and then continue other travels on an additional sheet?

You can attach the word file with the same contents as IMM 5562 Form like this (continue with other travels which they are not enough space in the form)

SUPPLEMENTARY INFORMATION

YOUR TRAVELS
1-Your Full Name:
Faimily Name:
Given Name:
Then, you insert a table with the columns like the form.

At the bottom of the page, you put IMM-5562 with the latest version (month-year)
