Unless you are asked you shouldn't redo your medical. Do you have a TRV already? If not it may be difficult to secure one with your whole family in Canada.

It may be very difficult to get approved because you have such strong ties to Canada there will be concerns that you aren't a temporary visitor. Has your spouse started the sponsorship process?

Yes, but processing time is too long, we have another small kid with me only 1.5 year old - she like joining her mother.

Spouses of PR and citizens often have a very difficult time getting approved for a TRV unless they have very strong proof that they have no intention of remaining in Canada and they are only coming for a temporary visit. When applying for a spouse as non-accompanying spouses often will need to remain separated while they wait for sponsorship. Depending on where you are located TRV processing can be quite long as well. I hope you do get approved for the TRV but I would not delay starting the sponsorship process.
