It wouldn’t matter if your mother was married on your birth certificate and you are stating she is single now. If it mattered, they would ask if she is divorced and ask fir the divorce papers or new marriage papers

Tnx for the reply. No happened married my mom . She's a single mom. Is she need an affidavit to proof that IShes my mother .and I'm using my father's surname not my mother . Do I need an explanation letter. Is it possible refuse my mom's application? Pls help

Don’t they ask for your birth certificate showing her name ? This would confirm it immediately. There should be some form of proof of relationship they ask for. You birth certificate should suffice

salmonarm is correct.

Your birth certificate will be the proof of your relationship to your mom. Your surname doesn't really mean much. (e.g. if you want to sponsor your dad, you cannot just use your name as proof. You still need your birth certificate)

And the "marriatal status" of your mom on you birth certificate isn't as important as yours and her name on it.
Since your application will state that she is "single" and not "divorced", it's better if you can attach a letter to explain that she got a CENOMAR (and if it's because she annulled her marraige explain that as well)
