FBI PCC - Question regarding FD-258 form entries

I got fingerprinted at a nearby police HQ. The lady who did my fingerprinting only supervised the signing of my signature and then signed herself. Other than that, she left me to fill the form. There are a lot of cells where I do not know what to fill. But i figured most of them do not apply to me. However, I am not sure of "ORI". Can I leave it blank? If not, what do I enter here?

I left the ORI Blank and it was no issue. I received my FBI report
