What do we do if the details of ex-wife is unknown and the child is in her sole custody and we don't know the child's current address?

Should this child be added in the family information form?

Yes, the child needs to be added. If you want to exclude the child, you will need to show proof that you have no access to the child, the custody papers, proof of trying to contact the ex to get the medical done. You will need a notarized letter confirming that the child will be excluded from the Family Class and you will never be able to sponsor him/her.
