If you are from India you can request certificate of DOB from Consulate General of India, they charge $45. They will ask you for your passport and if your parents name is on passport they will issue you the certificate with your parents names on it.

For yourself include copy of other documents which can show your rleationship to parents such as Educational documents, Passposrt and include affidavit along with explanation letter.
For your parents thy should also make affidavit for marriage certificate and explanation letter.

Do not send the actual form. Print the form into .pdf and send the .pdf out instead. Only send the form after they are validated and do not change them after it's signed.

We are a family unit of 4. My parents 2. Total 6. Should I add her?

Won’t they say something, scared to take any chances

Email the completed forms to them, They will print out and sign and then they can include all other required documents such as marriage certificate or Photos and send you courier. Do not get only signature pages from them because the paper quality and color can be different. Do not take any risk.

Does anyone know if I can sponsor one parent and one from in laws and how the financial requirement would like? Does that take into account undertaking not in effect? Which one takes precendence? The form first sent or based on signer vs cosigner?

Parents and Grandparents do not need Birth Certificates for application submission. If the marriage certificate is missing then ask them to make an affidavit and you can include explanation letter why marriage certificate is missing.
Note: You will need a birth certificate copy to show your relationship with parents.
